Initial Visit
55 Minutes/$195
This is the first visit to get to know you and truly listen to your struggles, goals and hopes for treatment outcomes. We’ll dive into your medical history, lifestyle, needed screenings or labs in order to develop an individualized health and longevity plan. You’ll receive questionnaires prior to your visit to complete on your own time, so we can use our whole time together getting right to your goals.

Simple Visit or Follow Up Visit
25 Minutes/$95
This type of visit is intended for follow up visits after your in depth initial visit, or if you have a straightforward, simple problem visit, like treatment of simple vaginitis, UTI, herpes outbreak, breast infection, etc.

A telehealth visit means you will see me either over an audio call or video call from your home. You will need a computer or cell phone for the visit. When you book a visit, you will be instructed to create a patient portal account through ChARM, which is the platform I use for visits. At the time of your visit, you will be able to join via audio or video through the patient portal.

I am an out of network provider, meaning I am not contracted with insurance providers. This is on purpose- because I never want to be held to the unrealistic standards insurance companies set that could potentially prevent me from giving the absolute best care that I can provide. Insurance companies add unnecessary red tape and barriers to care, and the whole mission at Ziva is to provide direct care to people without a middle man.
I understand that paying out of pocket, especially for those used to insurance based health care systems can be cumbersome for some families, and I am happy to discuss financing options or sliding scale options if cost is preventing you from accessing care with me.
I am also happy to provide you with a superbill that you can submit to your insurance company for reimbursement!

I am currently only licensed in Massachusetts, meaning I can only see patients who reside in Massachusetts and are in Massachusetts at the time of their visit. Stay tuned, as my licensing in more states may be available in the future!

No, I provide telehealth only visits. I strongly believe that telehealth is central to the future of health care delivery in the United States. It has the potential to significantly improve access to care, address disparities in health outcomes, and help make healthcare more equitable. Evidence suggests that telehealth provides comparable health outcomes when compared with traditional methods of health care delivery without compromising the patient–provider relationship, and it also has been shown to enhance patient satisfaction and improve patient engagement. Telehealth can better connect people to providers, help people better manage health conditions from home; and reduce the impact of structural barriers like transportation access, which interfere with people’s ability to get the health care they need. More widespread availability of telehealth can also allow for more timely access to care that can be provided remotely, as well as free up resources for care that must be provided in person, which is especially important in under-resourced health care systems.

Unfortunately, I am currently opted in as a medicare provider at my midwifery job which means that I cannot see you as self pay through Ziva if you have medicare. I can, however, help you via email find the right provider for you, and if you’re local you can see me in person at Brockton Neighborhood Health Center using your insurance!
